WebbProbably it's caused by UTBMap.kext. I have a similar issue. I see a lot of vm_shared_region_start_address() failed messages. However, it turns out to have no effect on macOS installation. I can see the installation GUI after removing UTBMap.kext and use UTBDefault.kext instead. Maybe, it's related to the 15 USB port limit.
